MyBlue Health has the lowest premiums in the BCBSTX lineup, and on paper the Silver CSR variants look like the deal of the year — $0 deductible, $10 specialist copays. Understanding Subsidies and Cost-Sharing Reductions (CSR)

Most Texans shopping BCBSTX through the marketplace qualify for two types of financial help:

1. Advance Premium Tax Credit (APTC)

Reduces your monthly premium based on household income and family size. Available at every metal tier (Bronze/Silver/Gold) for incomes generally between 100%–400% of the Federal Poverty Level (FPL) — and beyond that since the American Rescue Plan / Inflation Reduction Act subsidy expansions. Open Enrollment Period: November 1, 2025 – January 15, 2026. Plans effective January 1 if enrolled by December 15, otherwise February 1. For 2027 plan year, watch for Open Enrollment in late 2026.

Special Enrollment Period (SEP): Available year-round if you have a qualifying life event — loss of coverage, marriage, divorce, birth or adoption, moving, income changes that qualify you for subsidies, etc. You typically have 60 days from the event to enroll.

How do I check if my doctor is in-network?

Go to bcbstx.com/find-care and select the specific network name (not just "BCBSTX"). Networks to choose from: "Blue Advantage HMO," "Blue Advantage Plus HMO," "MyBlue Health," or "Blue Choice PPO." A doctor being in Blue Advantage HMO does NOT mean they're in MyBlue Health. Verify each network separately. Or call us and we'll verify for free before you enroll.

What's the difference between Blue Advantage HMO and Blue Advantage Plus HMO?

Blue Advantage HMO is a pure HMO — you must use in-network providers except for emergencies, and you typically need a referral for specialists. Blue Advantage Plus HMO has all the same in-network benefits but adds out-of-network coverage at a higher cost share. You can also self-refer to specialists under the OON benefit. Plus is worth the modest premium bump for members who want flexibility.

Are BCBSTX plans HSA-eligible?

Most Blue Advantage HMO and MyBlue Health plans are NOT HSA-eligible. BCBSTX does offer HSA-compatible plan variants in the Bronze tier for members who want to pair with a Health Savings Account. If HSA eligibility is important, tell your agent upfront — it narrows the plan selection considerably.

Does BCBSTX cover GLP-1 drugs like Ozempic, Wegovy, and Mounjaro?

Coverage varies by plan and by medical necessity (Type 2 diabetes diagnosis vs. weight management). Blue Advantage HMO members have reported Ozempic coverage; weight-loss-only prescriptions are often denied or require prior authorization. Check the plan's drug formulary (PDL) for your specific medication before enrolling if this matters to you.

Can I change plans outside Open Enrollment?

New 4 Tier Benefit Design

  • New requirement for CMS Standardized Plan Offerings
  • Available to On-Exchange plans
  • Drug lists are exactly the same for 4 Tiers and 6 Tier prescription plans
  • Some brands may be placed in generic Tiers and some generics may be placed in brand Tiers
  • 87.3% of prescriptions filled (2022 YTD) fall into Tiers 1 and 2 of a 6 Tier benefit structure
  • 43% of all medications on the formulary are in the first two Tiers of a 6 Tier benefit structure

Pharmacy Networks 

  • Contains a tiered network structure that categorizes in-network pharmacies into two buckets: Preferred and Non-Preferred
  • Members may pay a lower copay/coinsurance at an in-network Preferred pharmacy vs. using an in-network Non-Preferred pharmacy
  • Members can get up to a 90-day supply of medication from a Preferred pharmacy or a 30-day supply of medication from a Non-Preferred pharmacy
  • 90-day supplies are 3 times the 30-day retail copay via Preferred pharmacies and mail order through Express Scripts® Pharmacy
  • Select Independent Pharmacies may be included in a Preferred Pharmacy Network

Specialty Pharmacy Program

Specialty medications are used to treat conditions such as Multiple Sclerosis, Rheumatoid Arthritis, Cancer, and Cystic Fibrosis. Drugs may have special handling/storage requirements.

  • Medications are limited to a 30-day supply due to high cost and to prevent waste Self-administered products are standardly covered under the pharmacy benefit while physician-administered products are covered under the medical benefit
  • We deliver savings on specialty drugs for our clients and member through Prime’s Specialty Network offering
  • Accredo, our specialty pharmacy partner, has pharmacies across the country that allows drugs to be dispensed from the pharmacy location closest to the patient
