For 2026, Blue Cross Blue Shield of Texas offers three distinct individual HMO plan families — Blue Advantage HMO (statewide), Blue Advantage Plus HMO (statewide, with out-of-network flexibility), and MyBlue Health HMO (metro-focused, lower premiums). ⚠ Network warning for MyBlue Health
MyBlue Health has the lowest premiums in the BCBSTX lineup, and on paper the Silver CSR variants look like the deal of the year — $0 deductible, $10 specialist copays. But r/HealthInsurance moderators and Houston members consistently warn that the network is too narrow to be practical for most people. One Houston member reported only 7 hospitals in the entire metro took it, two of them county hospitals. Before enrolling in MyBlue Health, verify your doctor and hospital accept it — not just "BCBS."

Understanding Subsidies and Cost-Sharing Reductions (CSR)
Most Texans shopping BCBSTX through the marketplace qualify for two types of financial help:
1. Advance Premium Tax Credit (APTC)
Reduces your monthly premium based on household income and family size. Available at every metal tier (Bronze/Silver/Gold) for incomes generally between 100%–400% of the Federal Poverty Level (FPL) — and beyond that since the American Rescue Plan / Inflation Reduction Act subsidy expansions. Many Texans see their full premium covered by APTC.
2. Cost-Sharing Reduction (CSR)
This is the secret weapon. CSR reduces your deductible, copays, and out-of-pocket max — but only if you buy a Silver plan. If your income is between roughly 100%–250% of FPL, a Silver plan becomes dramatically cheaper to actually USE (not just buy). That's why MyBlue Health Silver 807 can have a $0 deductible — the CSR variant strips nearly all cost-sharing away for qualifying enrollees.

When and How to Enroll in BCBSTX
Open Enrollment Period: November 1, 2025 – January 15, 2026. Plans effective January 1 if enrolled by December 15, otherwise February 1. For 2027 plan year, watch for Open Enrollment in late 2026.
Special Enrollment Period (SEP): Available year-round if you have a qualifying life event — loss of coverage, marriage, divorce, birth or adoption, moving, income changes that qualify you for subsidies, etc. You typically have 60 days from the event to enroll.

Which BCBSTX plan is "best" for me?
It depends on three factors: (1) what your doctor accepts, (2) how much medical care you expect to use, and (3) your subsidy eligibility. If you qualify for CSR and your doctor takes MyBlue Health, Silver 807 is hard to beat on value. If you travel a lot or have specialists your regular HMO doesn't cover, Blue Advantage Plus gives you backup. Is MyBlue Health actually worth the savings?
For the right person, yes — specifically if you live in one of the covered counties (DFW, Houston, Austin, San Antonio, El Paso, Waco, Corpus Christi metros), your doctor is in the MyBlue Health network, and the hospital you'd use for emergencies participates. For anyone who travels, has kids needing pediatric specialty care, or wants broad access, Blue Advantage HMO is almost always worth the extra premium.
How do I check if my doctor is in-network?
Go to bcbstx.com/find-care and select the specific network name (not just "BCBSTX"). Networks to choose from: "Blue Advantage HMO," "Blue Advantage Plus HMO," "MyBlue Health," or "Blue Choice PPO." A doctor being in Blue Advantage HMO does NOT mean they're in MyBlue Health. Verify each network separately. Or call us and we'll verify for free before you enroll.
What's the difference between Blue Advantage HMO and Blue Advantage Plus HMO?
Blue Advantage HMO is a pure HMO — you must use in-network providers except for emergencies, and you typically need a referral for specialists. Blue Advantage Plus HMO has all the same in-network benefits but adds out-of-network coverage at a higher cost share. You can also self-refer to specialists under the OON benefit. Plus is worth the modest premium bump for members who want flexibility.
Are BCBSTX plans HSA-eligible?
Most Blue Advantage HMO and MyBlue Health plans are NOT HSA-eligible. BCBSTX does offer HSA-compatible plan variants in the Bronze tier for members who want to pair with a Health Savings Account. If HSA eligibility is important, tell your agent upfront — it narrows the plan selection considerably.
Does BCBSTX cover GLP-1 drugs like Ozempic, Wegovy, and Mounjaro?
Coverage varies by plan and by medical necessity (Type 2 diabetes diagnosis vs. weight management). Blue Advantage HMO members have reported Ozempic coverage; weight-loss-only prescriptions are often denied or require prior authorization. Check the plan's drug formulary (PDL) for your specific medication before enrolling if this matters to you.
Can I change plans outside Open Enrollment?
Only with a qualifying life event (see "Special Enrollment Period" above). You cannot switch from MyBlue Health to Blue Advantage mid-year just because you realized your doctor isn't in-network. This is the biggest reason to verify networks BEFORE you enroll, not after.
